Blizzard Awards 2006: Entry Form Check List

  1. Submitted On-line Official Entry Form

  2. Printed copy of the completed ENTRY FORM with signed ACCEPTANCE / CONFIRMATION FORM including Entrant signature , and as required, confirmation of Authorized Signing Person.

    Please ensure all information on the entry form is accurate.

  3. Complete list of On-Screen Credits or names with positions (for commercials and similar non-screen-credited motion pictures).

  4. Optional Entry Form Information by Category (max 30 words):
    1. Program Categories (A1-A14) SYNOPSIS ( especially for non-English language submissions) and/or Target Audience (Youth/Age Group) with Impact / Results; &/or, if applicable (particularly for Non-Commercial / Artist), creative approach, unique techniques &/or production challenges.
    2. Performer Categories (B1-B5) SCENE or CONTEXT - role (especially if Production Still not available) or story background.
    3. Craft Categories (C1-C12) PRODUCTION BACKGROUND - creative approach, unique techniques, &/or production challenges.
  5. TWO (2) VHS or DVD viewing copies of the motion picture (or the compilation/demo reel - maximum 30 minutes). Please note - All motion pictures selected for nomination will be required to submit a DVD or broadcast quality Betacam SP to the On Screen Manitoba office, at the expense of the Entrant. These must be delivered within 2 weeks of the announcements of Nominees.

    All viewing copies must be labeled with the following information:

    • Title:
    • Length:
    • Producer, Production Co.
    • Category(ies)
    • Contact Info.

    Example:

    • Title : “Bob's Sock Monkeys”
    • Length : 92 minutes
    • Producer : Taz Sawchuk, L'il Red Devil Productions
    • Category/ies : A1, A14, B2, C1-5
    • Contact Info : Robert Grant 555-5555 robert@monkeys.ca

    Only a maximum of 2 viewing copies of a Motion Picture are needed for Nominations Jury stage - regardless of the number of categories entered.

  6. Budget top sheet signed by producer if entering A1 or A2 Drama.
  7. Performer (B1-4) or Presenter (B5) headshot or photo from the production showing the actor in costume and make-up or the presenting team. If production still is not available, include a 30-word synopsis of the character's role in the cued scene, or the story context/ presenter impact.
  8. Complete press kit (optional).
  9. Entry fees if not paid on-line with VISA (see tally sheet*).
  10. Be sure to read the Blizzard Awards Rules and Regulations.
