Blizzard Awards 2006: Official Entry Form

EARLY ENTRY: Friday, October 14, 2005 - 5:00pm

FINAL ENTRY: Friday, October 28, 2005 - 5:00pm

How to Enter:

1. Determine category or categories:
   
   
   1. Program (Fiction or Non-Fiction)
   2. Performer (in front of the camera)
   3. Craft (individual to team entry)

2. Complete one online entry form for each motion picture and for each Craft and Performance entry.

1. Information provided on this Official Entry Form will be used verbatim in the Blizzard Awards 2006 program guide as well as for other promotional purposes as required for the event. Therefore, it is critical that you be accurate and complete.
1. Calculate the fee(s) by date submitted (See Early and Final Entry rates) and, if applicable, apply 10% bulk discount. Pay the appropriate ENTRY FEES using Visa online, or include a cheque or money order with the submission package.
2. Submit your entry online. Collect the support materials using the ENTRY MATERIALS check list.
3. PRINT, sign and/or collect Authorized Signing Person signature, and include signed a copy of the ENTRY FORM, with your submission package.
1. Entrant Signatures must be original on all documents submitted.
2. An Authorized Signing Person can send an authenticated eMail (see Definitions: that is, an eMail sent the individual with an eMail address that uses the individual's name.)
4. Deliver your submission package with signed Entry Form including all support materials and fees not paid online to On Screen Manitoba's offices by deadline.

INCOMPLETE AND/OR UNSIGNED SUBMISSIONS WILL NOT BE ACCEPTED.

This form must be submitted online and printed and included with your support materials and submitted to On Screen Manitoba.

All submissions must be received at the On Screen Manitoba Blizzard office by October 14, 2005 at 5:00pm for Early Entry or Friday, October 28, 2005 at 5:00pm for Final Entry.

THERE WILL BE NO EXCEPTIONS OR EXTENSIONS.
